Registration will be open until 7:00 pm

1st - 5th grade

Monday, August 17– Thursday, August 20, 2009

8:00 am – 2:00 pm

School Supplies and Spirit Shirts on sale

The following items will be needed to complete the registration process:

1. BIRTH CERTIFICATE

2. CURRENT SHOT RECORDS

3. SOCIAL SECURITY CARD

4. DRIVERS LICENSE OR IDENTIFICATION CARD

5. PROOF OF RESIDENCE
    a. Gas, water, or light bill - address must match I.D. or Drivers license

    b. Lease agreement-address must match I.D. or drivers license and

          all children must be listed on lease agreement.

If you are a guardian over the child you are enrolling you will need to go over to the Administration Building off Beltway 8 and Woodforest Blvd. and fill out papers before you can register the child. Go to the lobby and ask the reception for Guardianship papers.

If you have custody of your child, please bring a copy of your custody papers so they can be placed in your child's cumulative folder.

If you are living with a Galena Park resident and cannot provide your own proof of residency, you will have to have a notarized and approved affidavit in order to enroll your child. Please call the school for further information or come by to pick up an affidavit from the front office.
